Advance Care Planning (Free Info Session)
What does a well-lived life look like to you? What does a well-supported death look like to you? Please consider joining Life Transitions Doula- Colleen Corrigan, Retired Family Physician- Jay Federman, and Family Nurse Practitioner-Jennifer Phelabaum in an Advance Care Planning Informational.
Together we will demystify the process, directives, and additional considerations that often bog down the advance care planning experience.
What you can expect

Understanding the directives, when they may be applicable, resource support (living will, health care proxy, MOLST, DNR, etc)

Discussion regarding life sustaining treatments- the true facts and considerations (feeding tube, breathing tube, CPR)

Overview of end of life and after death care rights and options

Structured opportunities for values-based reflection
The program begins with a stand-alone Advance Care Planning informational session, open to residents and community members, followed by an optional multi-session series for individuals who wish to engage more deeply in the planning process with Doula, Colleen Corrigan.
